Information we collect

Personal Information
When you visit or use our digital services, we will collect personal information from or about you such as your name, email address, mailing address, telephone number(s), and a user name and password. We may also collect Employer ID number, company name, and fax numbers associated with your identification when you provide such information while using our digital services and where we believe it is reasonably required for ordinary business purposes.

Usage and Other Information
In addition to the personal information described above, we may collect certain information about your use of our digital services.  For example, we may capture the IP address of the device you use to connect to the digital service, the type of operating system and browser you use, and information about the site you came from, the parts of our digital service you access, and the site you visit next.   • Identity Verification. We may use the information we gather on the website for identity verification purposes, for example, to text users a code that they must enter on the website to verify identity when you access secure sites that we may offer from time to time. 

  • Contacting you Via Mobile Device. When you engage us to provide services you consent to us communicating with you on your mobile device you may provide. Such communications include but are not limited to calls, text messages, and email messages or other communication methods we may utilize now or in the future.  Calls or messages may incur fees from your mobile service provider. You further agree to receive commercial email messages at such email address from us, our vendors or advertisers unless and until you have elected not to receive such commercial messages by either contacting us as the address below or by responding to the opt out provisions of the message you receive. You understand that your wireless carrier’s standard rates apply to communications you receive via your mobile device. You represent that you are the owner, or authorized user of the mobile device on which communications will be received, and you are authorized to approve the applicable charges.

  • Improving Our Internet Content and Services. We use the information we gather to respond to any inquiries you make, operate and improve the functionality of our websites and services, and deliver the products and services which we provide.  

  • Legitimate Business Purpose. We may use the information we gather on the website for any other legitimate business purpose, as permitted by law. We may also use the information for any other purpose for which we receive your specific consent. 

We do not disclose your non-public personal information to third parties not assisting us with supplying your request for products or services without your authorization. However, we may share the personally identifiable information that we collect about you in the following ways:

  • Product and Service Delivery.  We share your information with service providers who help us in the delivery of the products and services you have requested.

  • Platform Functionality. We share your information with companies and individuals we employ to perform technical functions on our behalf. Understanding Cookies, Web Beacons and Other Tracking Technologies

We, or our service providers, and other companies we work with may deploy and use cookies, web beacons, local shared objects and other tracking technologies for various purposes, such as fraud prevention and monitoring our advertising and marketing campaign performance. Cowork Health, LLC may provide links to websites that are owned or operated by other companies ("third-party websites").  When you use a link digital to visit a third-party website, you will be subject to that website’s privacy and security practices, which may differ from ours. You should familiarize yourself with the privacy policy, terms of use and security practices of the linked third-party website before providing any information on that website.

Security

We use reasonable physical, electronic, and procedural safeguards that comply with federal standards to protect and limit access to personal information.  This includes device safeguards and secured files.
